PAINZ & Osgood Schlatters and Severs in children

Osgood Schlatters and Severs is a painful condition that affects growing children usually between the ages of 11 and 16. In the past it has been more common in boys than girls however now we see an even distribution between boys and girls mainly due to girls having more chance to play sport.

The pain that occurs at the front of the knee and the back of the heel happens when the growth plate is inflamed. In a growing child the growth plate is slightly detached as this is where the growth of the bone occurs in length and when the child is very active this already inflamed growth plate can become angered and become very painful immediately after sports. It can be as painful as a broken bone. Rest and cool compresses immediately after sport will always help. However the children that are sporty off and do not want to rest and asking them to have time away from sport is very difficult and emotionally traumatising for them if they’re not playing sport every day. So this painful condition is a series of events. Firstly the child is always very active and sporty secondly the child will be going through an intense growth fees for example if you look at the lines behind their knees you will see a few of these lines where there should only be one in an adult these growth lines were sure how much they have grown in recent months. And thirdly the Inability to rest I am for a few weeks to allow the inflammation to go down exacerbates the condition. There is usually a structural issue that makes the problem worse for example the feet leaning and can cause extra pressure on the calf muscle to attach with the Achilles tendon into the back of the heel bone. Children with high arches off and develop more pain in their knees due to the very strong pull of the anterior tibialis muscle into the toes which attaches to below the front of the knee and this is where the growth plate exists.

    My daughter is 11 and she plays basketball and tall for her age. She has started to get heel pain immediately after playing and we have to ice her heels when we get home and then she is ok. It happens only after basketball. She’s really good and she doesn’t want to stop playing. Please give us some advice. Thanks Patty

    My son is a very good golfer. He is 11 years old and has severe heel pain. We took him to the orthopaedic and they said he had severe disease and to not play sports. But he cannot take time off from golf as we feel he would get depressed. We have noticed his feet are leaning towards the inside and think this is make me the problem worse. What should we do?       Dear Chris, Severs condition is painful and affects the heels. The children suffer with severe pain especially after playing sport or during sport. You could use PAINZ a cooling oil and adhesive tape for the heels and we would advised bringing him to see Podiatrist Michelle Champlin at the Dubai Podiatry Centre for a corrective orthotic.     Can I bring my daughter there. She is really upset. She has been told to stop playing soccer and she lives for it. Maybe you guys can help her heels? She cries every day after school and we feel really bad. Thanks Carly

    Our son has really bad knee pain. He is a rugby player and he is going on a summer tour playing in South Africa and we are not sure if he will able to do it. He is devastated thinking he might Minot be able to go. He is really strong and takes a lot for him to complain about. He was told my the orthopaedic that it’s oscgood shlaters and he needs to rest. Is there anything you can help with? I know you are a foot clinic and treat feet but his pain is in his knees. He does not have flat feet. He has really high arches like his father. Please if there is anything you can suggest? Thank you Margaret

      Dear Margaret, children with very fast growth and flat feet suffer growth plate problems mainly at the back of their heels and children with high arches seem to suffer more at the front of their knees. The lower limb biomechanics are different for both. We can use an orthotic to change the foot and lower-limb biomechanics to reduce stress on the front of his knee.     Do you have any advice for my 11 year old son….he has flat feet and pain in his heels. I’m not sure if it is growing pains or flat feet or both. Thanks Jenny

    Our 13 year old son has grown a lot in the last 2 months and his feet look like they are flattening. He has knee pain and heel pain and plays soccer everyday. He limps after dinner but he can play football without pain but after it hurts him a lot. Is this normal? Should we stop him playing soccer? Thanks so much for any suggestions, Anne

      Dear Anne, it depends on the severity of his pain if he should stop playing soccer. You could bring him for assessment with Podiatrist Michelle Champlin.
