Foot corns can cause a lot of pain that can often times be debilitating. It is important to decipher which type of corn you have first as a lot of corns cannot be removed by simply scraping them away, this can often worsen the pain.

You can get many types of corns such as hard corns, soft corns and seed corns. This article will focus on Heloma milliare, aka ‘seed corns’.

Seeds corns are very, very painful. They look small, diamond-shaped and are more painful than a large hard corn. The reason for the extra pain is not due to the hard skin of the corn, it is what is causing it underneath.

Seed corns are not caused by pressure, they are caused by an overactive nerve ending. So, just like you can have freckles on your arms, it is the same thing but not with melanin, it is with keratin. The overactive nerve ending that is under the skin creates an overgrowth of keratin. The seed corns look like a bunch of little diamonds but it is actually the overactive nerves underneath that create this pain.

If a doctor misdiagnoses seed corns as a ‘hard corn’ and tried to shave off the hard skin to make it better this would worsen the pain. So by literally drilling out these tiny little corns it is not going to make the pain go away and in fact it actually can make it worse.

Our solutions for a hard corn and seed corn are different. For a hard corn, what we recommend to prevent a this from forming is to wear a custom insole that lifts the arch of your foot and takes the pressure away from the area which is the root of the cause.

However, for seed corns, we recommend seeing the dermatologist because they may be able to give you a cream that is a keratolytic that actually thins your skin and makes it more supple and baby-like and helps to reset the nerves underneath.
Also, they might be able to give you a vitamin A retinol cream, steroid cream or urea-based cream.

I highly recommend using vitamin A retinol cream onto the pad of your foot – bear in mind 1 skin cycle is 28 days, so you will need to apply this cream for 3 skin cycles (3 months) before you notice an improvement. It is going to be a slightly long process.

Additionally, I recommend wearing very thick and fluffy socks that will soften the pressure on your feet instead of thin socks or no socks at all. Even if you wear soft shoes it will still be an issue. Fluffy , thick socks will greatly ease the pain, such as Thorlos tennis or golf socks as they have thick padding.

Most importantly, the nerve endings creating the seed corns need to calm down. This is genetic so someone else in the family mostly likely has it and it normally also is created by having high foot arches. So, with high arches the nerve endings are very, very hyperactive and it can lead to these seed corns.

To conclude, scratching, scraping, or filing the seed corns it will not do anything, in fact you are just going to irritate the nerves more. I recommend consulting with a dermatologist for their professional opinion and diagnosis. The goal is to make the skin soft and baby-like using a retinol or a vitamin A high strength cream on your feet every night for 3 months and wear very soft, fluffy socks by brands such as Thorlos which you can purchase from Amazon (linked below). These 2 steps will really, really help.

This is the band of plantar fascia that we have underneath our foot and it attaches into the heel area here on the foot.

We’re going to help to reduce the tensile stress and impact of the fascia to the heel bone by using this one single sheet of plantar fascia tape.

Now we’re also going to combine it with toe socks and I’ll explain in a moment why that’s very important.

Here we keep the toes aligned and independent because here you can see the attachment of the fascia into each individual toe.

So we want to keep the toes stretched nice and straight and we use that by combining it with independent toe socks.

It is quite common for children to intoe.

There are three main reasons why they can do it.

In this case the intoeing is coming from the hips.

So the hips are internally rotated and the entire leg is turning in.

This second case is coming only from the feet, so the feet only are turning in.

In the last case is where the ankles are leaning outwards and that is turning the feet.

This is from the ankles, this is from the foot and this is from the hips.

There are a few other reasons why the child can intoe as well but these are the three main types that we see here in the clinic.

Each one of these requires a different type of insole inside the shoe.

The insole inside the shoe in all three cases are very different to each other even if the intoeing all looks similar to an outsider.

So this is why it is very important to come in and have the podiatrist check which type of intoeing the child has.

This is a contrast foot bath.

Using warm water of 42° and cold water 22°.

The benefit of using a contrast foot bath is that it helps with swelling and foot injuries.

Now you put the feet into warm water for 1 minute then cold water for 1 minute and you will repeat 5 times.

This brings oxygenated blood from the heart to the feet.

The cold water helps to take it away from body.

The best idea is to do this process next to the kitchen sink.

Use a thermometer so you can get the right temperature. This one is quite cheap from Amazon.

And use a Samla box that fits both feet nicely.

We’re fitting an orthotic today for a patient with a bunion.

We have the Wave orthotic system which is what we use for measurements of bunions.

We take photos and the patient also gets an ink less impression of the feet at the start of the treatment.

It is ink less so it does not leave any marks on the feet for the convenience of the patient.

As you can see it will be assessed at the end of the treatment. We compare this before and after treatment.

As well as impressions of the feet we take outline drawings of the feet so that we can check these before and after.

It is also a good tool for measuring the feet when it comes to footwear fitting.

The patient can choose the colour of orthotic, here they have chosen Hatta fort colour.

Now here are the finished products.

The orthotics are going underneath the foot to get the perfect fit.

As you can see, the big toe has straight alignment.

It is very useful with toe socks so that the toes are able to go back to their straight position.

And we are fitting them in a wide barefoot style shoe that accommodates all the toes to a straight position.

The patient will now have a little walk in the shoes to make sure they are comfortable.

They are quite steady on their feet and this is the finished product.
