We’re fitting an orthotic today for a patient with a bunion.

We have the Wave orthotic system which is what we use for measurements of bunions.

We take photos and the patient also gets an ink less impression of the feet at the start of the treatment.

It is ink less so it does not leave any marks on the feet for the convenience of the patient.

As you can see it will be assessed at the end of the treatment. We compare this before and after treatment.

As well as impressions of the feet we take outline drawings of the feet so that we can check these before and after.

It is also a good tool for measuring the feet when it comes to footwear fitting.

The patient can choose the colour of orthotic, here they have chosen Hatta fort colour.

Now here are the finished products.

The orthotics are going underneath the foot to get the perfect fit.

As you can see, the big toe has straight alignment.

It is very useful with toe socks so that the toes are able to go back to their straight position.

And we are fitting them in a wide barefoot style shoe that accommodates all the toes to a straight position.

The patient will now have a little walk in the shoes to make sure they are comfortable.

They are quite steady on their feet and this is the finished product.

With a flat foot most of the time the ankles are leaning inward as you can see here.

When the ankles are leaning in we can do two different types of insoles.

This is a corrective insole and it tilts the ankles straight. So from here to straight position.

And then this is an accommodative insole. This insole still allows the ankles to lean in but it stops them from getting worse.

What’s the difference between the two?

Most of the time the patient’s foot is able to be tilted straight.

But, some of the time if there is arthritis, surgery or restriction in the joint movement and we cannot make the ankle straight then we would use the accommodative insoles.

This corrective insole is the most popular because most of the ankles that we see here in the clinic can be tilted straight.

So we like to get the ankle from here to straight position using corrective insoles or keep it held with supportive accommodative insoles.

This patient is halfway through orthotic therapy for flat feet and he is happy with the results.

He is traveling and we have asked him to wear toe socks and orthotics on his holiday.

We are going to use it in his new shoes. We have asked him to remove the liner of his new shoes and replace with his new orthotics.

Here you can see him removing the existing insole and replacing with the corrective flat foot insole to lift his arches.

With his second pair of shoes he is also removing the existing flat liner and replacing with his colour matching corrective orthotics for his shoes.

This is the desired foot alignment we would like to achieve with this patient.

Are here we can see the ankles are straight , and the big toes have their straight alignment.

The arches are also engaged and lifted.

As we remove the correct orthotic, the ankles start to lean-in again.

This creates an issue with the big toe. It’s important to try and keep the ankles straight with the corrective orthotic to achieve the desired results with the bunions.

Without it ankles lean-in, arches collapse and the toes move away to the sides.

Removing the silicone from the toe socks, we will again align the ankles and toes into the desired position.

The important part is to achieve straight ankles to have straight alignment of the big toes.

When the ankles lean-in, see how the big toes move away to the sides.

Removing the toe socks, we can see if the alignment issues much better. We can see the ankles leaning-in when the patient relaxes making the toes move to the sides again.

Look how we tilt the ankles back to straight. We achieve a fantastic result with the alignment of the big toe.

Angles are so important when it comes to treatment of bunions.

This patient is suffering from bruised toenails and hard skin on the end of the toes.

As you can see from this insole, her toes are right up at the end of the shoe. This is why she has bruising and calluses, and is losing the second toe nail.
.وكما ترون من هذا النعل، فإن أصابع قدميها موجودة في نهاية الحذاء. ولهذا السبب تعاني من كدمات ومسامير، وتفقد ظفر إصبع قدمها الثاني

And the first toe nail on this side is beginning to separate from the skin. That is because the toes are being squeezed and pushed right up to the end of the shoes, so the shoe is too small.
.ويبدأ أول ظفر في إصبع القدم في هذا الجانب بالانفصال عن الجلد. وذلك لأنه يتم الضغط على أصابع القدم ودفعها حتى نهاية الحذاء، وبالتالي يكون الحذاء صغيرًا جدًا

You can do this yourself by removing the insole from your sports shoes and if you don’t have a centimetre and a half of free space where the toes don’t leave any impression, then the shoes are too small.

If you suffer from corns and callus in a very specific area of the foot, for example here under the ball of the foot, it is typically due to an imbalance in the 3 foot arches.

The 3 arches are the medial arch, lateral arch and the transverse arch.

When the transverse arch collapses, the second, third and fourth metatarsal heads of the bones drop down and start pushing through the skin.

This makes the skin toughen up, harden, and become pure almost like toe nail.

This is why corns and calluses form in this area. We can fix that by lifting the ball of the foot back up again with insoles to be worn for about 3-6 months.

Flat feet can cause issues with running and walking in some children

‏If you notice when the child is running away from you that they are leaning forward and they are wiggling when they are running, it could mean that we need to put something inside their shoes that help their body alignment.

إذا لاحظت أن الطفل يميل إلى الأمام ويتمايل عندما يركض، فقد يعني ذلك أنه قد نحتاج إلى وضع شيء داخل حذائه يساعد على تحسين توازن جسمه

 

‏When the child is running towards you, you may notice that the feet are turned more outwards. This puts more pressure on the front of the body making it easier to fall.

عندما يركض الطفل، قد تلاحظ أن القدمين تتجهان إلى الخارج أكثر ممّا يشكل ضغطًا أكبر على الجزء الأمامي من الجسم ويزيد احتمال التعرض للسقوط

 

‏We would put something inside the school shoes and inside the sports shoes that lifts the arches and brings the feet in to a straighter position, putting the bodyweight further back allowing the abdominal muscles to be engaged when they are running.

سنقوم بوضع شيء داخل حذاء المدرسة وحذاء الرياضة ليرفع الأقواس ويجعل القدمين في وضع مستقيم أكثر، مما يسمح بتحميل الوزن إلى الوراء أكثر ولعضلات البطن المشاركة بعملية الركض

 

‏If you think your child needs something, bring them to the clinic with their school shoes and sports shoes. If there is a requirement we will put it in to their shoes making the child more efficient at running and school.

‏For children with flat feet we were discussing putting something inside their sports shoes and school shoes that helps to align their feet and ankles.

بالنسبة للأطفال ذوي الأقدام المسطحة، نوصي بوضع شيء داخل أحذيتهم الرياضية والمدرسية ليساعد على  محاذاة القدمين والكاحلين

‏This is what we would put inside this girls shoes. It lifts the arch and tilts the ankle back to straight again.

هذا هو ما سنقوم بوضعه داخل حذاء الفتاة. يرفع القوس ويعيد الكاحل إلى الوضع المستقيم

‏And inside the school shoes we have coloured matched it so you don’t see anything inside the school shoes.

أمّا بالنسبة إلى أحذية المدرسة، فقد قمنا بتنسيق اللون لضمان عدم ظهورها من داخل الحذاء

‏And when we remove it you can see again that we are lifting the arch and tilting the foot so we can get the ankle and foot back to a straight position.

عندما نقوم بإزالته، ترى كيف قمنا برفع القوس وإمالة القدم، مما ساعد في إعادة الكاحل والقدم إلى وضع مستقيم.

‏This helps to grow greater confidence and faster running without the chance of falling down.

يساعد ذلك على إعادة الثقة وإمكانية الركض بسرعة أكبر دون خطر السقوط

Young children with flat feet tend to fall down quite easily and have difficulty running quickly

غالبًا ما يقع الأطفال ذوي الاقدام المسطحة بسهولة أو يصعب عليهم الركض بسرعة

‏The reason for that is when the arches are flat the ankles lean in and the child finds it very difficult to run and walk in that position.

وهذا لأن الأقواس مستقيمة فيميل الكاحلين ويصبح من الصعب على الطفل الركض أو المشي في هذه الوضعية

‏So, they find a better way to run with their feet pointed outwards.

لذا يصبح من الأسهل لهم الركض وأقدامهم موّجهة للخارج

‏When that happens it tends to tilt the upper body forward so in turn they can fall forward quite easily.

عند الركض بهذه الطريقة، يميل الجزء العلوي من الجسد إلى الأمام فيزداد احتمال الوقوع

‏So, we would put something inside their school and sports shoes that lifts the arches and tilts the ankles back to straight giving the child better confidence when running.

لذلك نقوم بوضع شيء داخل الحذاء المدرسي و الرياضي ليرفع الأقواس ويعيد الكاحلين للوضع المستقيم وبالتالي إعادة الثقة للطفل خلال الركض
