All of these show the left foot. You can see the left foot is leaning in, leaning out, and on this picture the left foot is straight. When we have a tilted pelvis, the foot leans in. It creates swelling on the inside of the heel. When we have a tilted pelvis that is higher up and moves back, the foot leans to the outside and the swelling is outside the heel. When we have a pelvis that is straight, both feet and Achilles are straight then we don’t have any swelling on the heel bone.

A great tip for preventing issues with toes such as fungus in between the toes, crooked toes, corns and bursae is these special toe socks which I highly recommend.

Toe socks help to keep the toes separate and alleviates the moisture from building up in between the toes which typically causes fungal infections.

‎ تساعد جوارب أصابع القدم على بقاء أصابع القدم منفصلة وتخفيف الرطوبة من التكوّن بين أصابع القدم والتي تسبب عادةً الالتهابات الفطرية

 

Also, it is common for people to have toes longer than their big toe, such as the second or third toes and wearing normal socks can make these longer toes bend slightly. This can create some bursars, swellings and corns on the top. To prevent this, wearing toe socks would help with any discomfort and friction.

‏I am going to discuss the difference between a flat foot and an arched foot.

‏‎سوف أناقش الفرق بين القدم المسطحة والقدم المقوّسة

‏It’s interesting to see that the bones in the flat foot in this model compared to the bones on this arched foot model are identical and there’s no difference between the bones. But what’s different is the position of the bones. So just liked crooked teeth you can get a brace to straighten it and change the alignment of the teeth without changing the integrity of the individual tooth.

As you can see from these feet, this is what a flat foot looks like in real life, this is what arched feet look like in real life, but let us see how to identify if a flat foot can be raised and arched again.

This fungal infection on the toenail has been active for more than 20 years according to the patient.

Some flat feet are normal, some flat feet are not normal.

When you see a child walking with abducted feet pointing outwards as you can see here, that type of flat foot means the ankles are falling inwards and they have to walk with their feet abducted.

 عندما ترى طفلاً يمشي بأقدام ملتوية متّجهة للخارج كما ترون هنا، فإن هذا النوع من القدم المسطحة يعني أن الكاحلين يسقطان من جهة الداخل وعلى الأطفال المشي بأقدامهم الملتوية

 

This can lead to more falling and difficulty running.

 هذا يمكن أن يؤدي إلى مزيد من الهبوط وصعوبة في الجري

To solve this, we would put something underneath the foot and inside their shoes so that it lifts the arches up and makes their feet go back to being straight.

لحل هذه المشكلة، نضع شيئًا أسفل القدم وداخل أحذيتهم بحيث يرفع الأقواس للأعلى ويجعل أقدامهم تعود إلى الوضع المستقيم

Firstly, we can see a two year old child with a wide stance, arms pointing outwards and their flat feet are abducted with the ankles leaning in.
‎أوّلًا، نرى هنا طفلًا بعمر السنتين بوقفة عريضة، الذراعين موجهتين للخارج وقدميه المسطحتين ملتويتين مع انحناء داخلي للكاحلين

 

 

 

 

 

 

 

Next, an example of a six year old child shows the feet have become more flat. Six year old children develop excellent leg muscles but will complain of being tired after walking long distances.

In the example of a 10 year old child, their pelvis is tilted, the feet are abducting even more and they will start to show up issues when they are running.

Lastly, the example of a 16 year old child has excellent muscle tone in the legs but the feet have abducted even more as the feet reach their maximum flatness.

This is why we don’t want any child to not be involved in their sport or activities and we like to lift the foot arches and get the ankles back to straight as young as possible.

There are many different types of bunion which is shown in this diagram.

In this example we can see this bone is straight however on this other one we can see there is a bony lump that has grown on the side.

On this other structure the bone is straight but the head is enlarged.

Also, for this bunion the bone has twisted round.

‎كذلك بالنسبة لهذا التورم، العظمة هنا ملتوية

 

 

 

 

Here, the capsule that holds the joint together has swollen.

‎هنا ، تورّمت المحفظة التي تربط المفصل معًا

 

 

 

 

Lastly, on this one the joint has displaced and moved out to the side and this is the most common type of bunion we see in the clinic.

There are ones that require surgery and there are ones that don’t require surgery so it is important to find out which type of bunion you have.

In-toeing

‎الانحراف الداخلي لأصابع القدم

 

In-toeing is where one or both feet tend to turn in towards each other.

‎يحدث الانحراف الداخلي لأصابع القدم عندما تميل إحدى القدمين أو كلاهما نحو بعضهما البعض

 

 

This is called penguin foot, the whole leg turns in. The. In-toeing originates from the pelvis area.

‎هذا ما يسمّى بقدم البطريق حيث تميل الرجل بأكملها نحو الداخل. يبدأ هذا الانحراف من منطقة الحوض

We can put something inside the shoe to turn the feet back towards the outside again.

‎يمكننا وضع شئ داخل الحذاء لإعادة القدم نحو الخارج مجددًا

 

 

 

This is called ‘pigeon foot’ and that is where the big toe only pulls in, but when the child is wearing the shoe it can pull the whole leg and foot inside.

‎هذا ما يسمّى بقدم الحمامة وفيه يميل أصبع القدم الكبير نحو الداخل. إلّا أنه عند ارتداء الحذاء، تميل رجل وقدم الطفل كاملة نحو الداخل

 

 

 

 

This is called ‘banana foot’ and that is where the foot is actually curved like a banana.

‎هذا ما يُسمّى بقدم الموزة، أي حين تبدو القدم مقوسة كشكل الموزة

Again, we put something inside the shoe to pull the feet back out again.

‎في مثل هذه الحالة أيضًا، نضع شيئًا داخل الحذاء لتصحيح شكل القدم

 

 

These are the 3 most common types, there are other types that we see here in the clinic but these are the most common types.

The transverse arch of the foot is a very important foot arch.

القوس المستعرض للقدم هو قوس مهم جدًّا

‏It sits underneath the ball of the foot just behind the toe area and it is showing where the metatarsal bones from the mid to forefoot sit. 

يقع القوس المستعرض تحت كرة القدم خلف منطقة إصبع القدم  ويظهر مكان تواجد عظام مشط القدم من منتصف القدم الى المقدمة 

‏If the transverse arch is collapsed as you can see in this photo here it can give a pain in the forefoot. Also, it can lead to corns, calluses, bunions and give a burning sensation of the feet as well.

إذا هبط القوس المستعرض كما ترون في هذه الصورة هنا، من الممكن أن يُحدث ألم في مقدمة القدم.  كذلك، من الممكن أن يؤدي الى ظهور مسامير، وتصلّبات وأورام ملتهبة وشعور بالحرق في القدم

That is why we like to lift the transverse arch back up again as you can see from this photo here so that the transverse arch and all the metatarsals are back up to where they are supposed to  be.

Heel mosaic Veruca / الثؤلول الأخمصي الفسيفسائي 

‏This is a picture of a heel on the foot which is pink, supple and totally healthy. This is after they had treatment for mosaic warts.

‏Mosaic warts are spread around just like a mosaic tiled floor is laid out. We prefer to treat mosaic wart quite quickly because of how fast they spread.
