‎الانحراف الداخلي لأصابع القدم

 

In-toeing is where one or both feet tend to turn in towards each other.

‎يحدث الانحراف الداخلي لأصابع القدم عندما تميل إحدى القدمين أو كلاهما نحو بعضهما البعض

 

 

This is called penguin foot, the whole leg turns in. The. In-toeing originates from the pelvis area.

‎هذا ما يسمّى بقدم البطريق حيث تميل الرجل بأكملها نحو الداخل. يبدأ هذا الانحراف من منطقة الحوض

We can put something inside the shoe to turn the feet back towards the outside again.

‎يمكننا وضع شئ داخل الحذاء لإعادة القدم نحو الخارج مجددًا

 

 

 

This is called ‘pigeon foot’ and that is where the big toe only pulls in, but when the child is wearing the shoe it can pull the whole leg and foot inside.

‎هذا ما يسمّى بقدم الحمامة وفيه يميل أصبع القدم الكبير نحو الداخل. إلّا أنه عند ارتداء الحذاء، تميل رجل وقدم الطفل كاملة نحو الداخل

 

 

 

 

This is called ‘banana foot’ and that is where the foot is actually curved like a banana.

‎هذا ما يُسمّى بقدم الموزة، أي حين تبدو القدم مقوسة كشكل الموزة

Again, we put something inside the shoe to pull the feet back out again.

‎في مثل هذه الحالة أيضًا، نضع شيئًا داخل الحذاء لتصحيح شكل القدم

 

 

These are the 3 most common types, there are other types that we see here in the clinic but these are the most common types.

‎هذه هي الأنواع الثلاث الأكثر شيوعًا، الّا انّنا نرى العديد من الأنواع الأخرى في العيادة

The transverse arch of the foot is a very important foot arch.

القوس المستعرض للقدم هو قوس مهم جدًّا

‏It sits underneath the ball of the foot just behind the toe area and it is showing where the metatarsal bones from the mid to forefoot sit. 

يقع القوس المستعرض تحت كرة القدم خلف منطقة إصبع القدم  ويظهر مكان تواجد عظام مشط القدم من منتصف القدم الى المقدمة 

‏If the transverse arch is collapsed as you can see in this photo here it can give a pain in the forefoot. Also, it can lead to corns, calluses, bunions and give a burning sensation of the feet as well.

إذا هبط القوس المستعرض كما ترون في هذه الصورة هنا، من الممكن أن يُحدث ألم في مقدمة القدم.  كذلك، من الممكن أن يؤدي الى ظهور مسامير، وتصلّبات وأورام ملتهبة وشعور بالحرق في القدم

That is why we like to lift the transverse arch back up again as you can see from this photo here so that the transverse arch and all the metatarsals are back up to where they are supposed to  be.

Heel mosaic Veruca / الثؤلول الأخمصي الفسيفسائي 

‏This is a picture of a heel on the foot which is pink, supple and totally healthy. This is after they had treatment for mosaic warts. 

هذه صورة لكعب القدم، لونه زهريّ، مرن وصحيّ. هذه الصورة بعد أن تمت معالجة الثؤلول الفسيفسائي  

‏Mosaic warts are spread around just like a mosaic tiled floor is laid out. We prefer to treat mosaic wart quite quickly because of how fast they spread. 

تنتشر الثآليل الفسيفسائية تمامًا مثل الأرضية المكسوّة بالفسيفساء. نحن نفضّل معالجة الثآليل الفسيفسائية بسرعة  وذلك بسبب سرعة انتشارها

As you can see, after the treatment there is absolutely no scarring or sign of where the mosaic warts once were.

I’m going to show you four different types of  basic nail infections. 

سأريكم ٤ أنواع مختلفة من التهابات الأظافر 

 

‏The first one is when you have a skin fungal infection that spreads underneath the nail plate and infects the nail from underneath and upwards. It creates this wavy surface underneath the nail plate.

النوع الأوّل هو عندما يكون لديك التهاب فطري جلدي ينتشر تحت صفيحة الظفر وتصيب الظفر من الأسفل حتّى الأعلى مما يخلق هذا السطح المموّج تحت صفيحة الظفر 

 

The second type is when the nail actually has trauma its self. This gets the fungal spores into the nail plate and infects the nail plate.

النوع الثاني هو عندما يُصاب الظفر بأذى أو رضخ ممّا يُدخل الجراثيم الفطرية إلى صفيحة الظفر فتصاب بالتهاب

 

 

The third type that we see here is when the nail has a root infection from a broken cuticle and it has a fungal infection in the root that has spread all along the length of the toenail.

النوع الثالث الذي نراه هنا هو عندما يكون الظفر ملتهبًا من الجذور بسبب انكسار الجليدة وانتشار العدوى على طول ظفر القدم

 

 

Lastly, this type of fungal toenail infection is the most common. This is where nail polish has been worn for too long, once the nail polish has been removed there is a white crustiness on the top of the nail which is caused from wearing nail polish for too long and fungal spores have had a multiplication in the space of the nail plate and the nail polish. 

أخيرًا ، هذا النوع من التهاب أظافر القدم الفطرية هو الأكثر شيوعًا.  هذا هو المكان حيث تمّ وضع طلاء الأظافر لفترة طويلة، بمجرّد ازالة طلاء الأظافر ، تظهر قشور بيضاء على الجزء العلوي من الظفر بسبب وضع طلاء الأظافر لفترة طويلة جدًّا وتكاثر الجراثيم الفطرية في المساحة ما بين صفيحة الظفر وطلاء الأظافر

In this picture, this is a perfect example of a toenail which looks clear, healthy and shiny.

نرى في هذه الصورة المثال المثالي عن ظفر إصبع القدم خالٍ من الشوائب وصحيّ ولامع

 

‏However, nine months ago there was a fungal infection from the root to the tip of the nail, as well as a fungal infection from the nail plate and the skin underneath. 

إلّا انّه من تسعة أشهر، كان هناك التهاب فطري من جذر الظفر وحتّى رأسه إضافةً إلى التهاب فطريّ في صفيحة الظفر وأسفل الجلد

 

This is why we use a very specific type of treatment for this fungal nail infection so that we can get the fungal nail to become a very clear and shiny nail again.

لهذا السبب نقوم باستخدام نوع محدد من العلاج لهذا النوع من الالتهاب الفطري فنتمكّن من إعادة الظفر الفطري إلى ظفر خالٍ من الشوائب ولامع مجدّدًا

In this photo you can see the skin looks dry, peeling and flaking.

في هذه الصورة، يمكنك رؤية الجلد جاف ومتقشّر ومتفتّت 

‏It is common to try to make the skin go back to soft and supple again by moisturising the skin but this area of the foot is a fungal infection.

من الشائع محاولة إعادة الجلد إلى ملمسه الناعم والطري وذلك من خلال ترطيبه. إلّا أن هذه المنطقة في القدم هي التهاب فطريّ

‏As you can see if you look closely at it there are some small brown and red spots that swell that gives this type of fungal infection away.

عند النظر بتمعّن إلى هذه المنطقة، نرى بقع صغيرة بنّية وحمراء اللّون تتوّرم وهي التي تؤكد وجود الالتهاب الفطري

There are different types of fungal infection that affect the skin on the feet so it’s important we identify which type of fungal infection it is.

هناك انواع مختلفة من الالتهاب الفطري والتي تصيب جلد القدم، لذلك من المهم جدًّا معرفة نوع الالتهاب

 

‏The correct course of treatment will make the skin go back to being normal and soft.

This diagram shows a healthy and regular nail that has an intact cuticle. The nail is cut straight across and filed slightly at the edges. This will limit the potential for fungus to get inside the nail and in to the nail root.

يظهر هذا الرسم التوضيحي ظفرًا صحيًّا وطبيعيًّا مع جليدة سليمة. الظفر مقصوص بشكلٍ مستقيم في الأعلى وممتلىء قليلًا عند الأطراف. هذه الطريقة تحد من احتمالية دخول الفطريات في الظفر وفي جذوره

 

However, if the nail cuticle is removed it leaves the root at risk of developing a fungal infection. Also, if the nail is trimmed down too far at the sides it can lead to a fungal infection which can grow in the nail sulcus. 

إلّا أنّه في حال نزعت هذه الجليدة، تصبح الجذور عُرضة لالتهاب فطري. أيضًا في حال تقليم الظفر بشكل مفرط عند الجوانب، يمكن أن يُحدث ذلك التهاب فطري في تلم الظفر

 

This is an example of a toe without a nail. The nail sulcus is like a strong type of skin, similar to a special train track which supports the toe nail. If that gets trimmed away the nail can collapse and curl. 

هذا مثال عن اصبع قدم من دون ظفر. يشبه تلم الظفر نوع قاسٍ جدّا من الجلد يشبّه بممر القطار الذي يحمل ظفر إصبع القدم. في حال أُزيل هذا التلم بشكلٍ كامل، قد يهبط الظفر ويلتف

 

Another example shows a toenail that is cleaned too well because the cuticle has been removed which again can lead to a fungal infection. 

مثال آخر يُظهر ظفر إصبع القدم منظّف بشكلٍ مفرط بحيث نُزعت الجليدة ممّا قد يؤدّي الى التهاب فطري

 

On the other hand, the cuticles can overgrow and they can be pushed back and the excess trimmed. Although, the actual cuticle itself which we can in this example should never be removed.

في المقلب الآخر، يمكن للجليديات أن تنمو بشكلٍ مفرط وأن تُدفع إلى الوراء وتُقلّم الزوائد. أمّا بالنسبة إلى الجليدة في هذا المثال، فلا يجب إزالتها أبدًا

These are a few very simple and easy steps for a healthy nail. 

هذه بضع خطوات بسيطة جدًّا وسهلة للحصول على أظافر صحيّة

 

Firstly, keep the cuticle intact and never trim and remove it. When cutting the toenails, cut the nail straight across and file the nail slightly at the edge.

أوّلًا، حافظ على الجُليدة سليمة ولا تقم أبدًا بقصّها أو إزالتها. عند قص أظافر القدم، قم بقص الظفر بشكل مستقيم ثم قم ببرد الظفر قليلًا عند الطرف

 

Never cut the nail too far down at the sides because there is a special type of skin at the side called the ‘nail sulcus’ that supports the nail. If the nail is cut down too much at the sides it will collapse and curl.

لا تقم بقص الظفر كثيرًا من الجانبين لأن هناك نوعًا خاصًّا من الجلد على الجانب يُسمّى ” تلم الظفر” الذي يدعم الظفر. إذا  تمّ قص الظفر كثيرًا على الجانبين، سيهبط الظفر ويلتف

 

Also, if the cuticle is removed it can lead to fungal infection down in the root creating a fungal nail. 

كذلك إذا تمّت ازالة الجليدة فمن الممكن أن يتسبب ذلك بعدوى فطرية أسفل الجذور ممّا يخلق ظفرًا فطريًّا

This diagram shows some of the changes that happen in the legs because of flat feet.

يُظهر هذا الرسم التوضيحي بعض التغيرات التي تحدث للساقين بسبب القدمين المسطحتين 

 

 

In this first example, the arches drop down so the person will find it difficult to walk with their feet straight. So, they abduct their feet and roll off of the inside of their feet which is why the knees are facing outwards. 

في المثال الأوّل، تهبط أقواس القدم ليصبح من الصعب على الفرد المشي بطريقة مستقيمة. لهذا السبب، يبعدون أقدامهم ويديرونها تجاه الداخل ممّا يجعل الركبتين مواجهتين للخارج

 

 

 

If they try and walk with their feet straight this diagram shows what will happen, the knees end up pointing towards each other and we call it ‘X legs’ because the legs tend to look like the English letter ‘X’.

في حال قاموا بالمحاولة والمشي بطريقة مستقيمة، نرى من خلال هذا الرسم التوضيحي أنّه سينتهي الأمر باتجاه الركبتين نحو بعضهما البعض. هذا ما يُسمّى بالأرجل X، إذ إنّها تبدو مشابهة للحرف X في اللّغة الإنجليزية

 

 

 

When we have one leg that is straight and the foot arch is up and the other foot arch is collapsed, that knee will face towards the other one. We call that a ‘K leg’ because it looks like the English letter ‘K.’

عندما تكون ساق واحدة مستقيمة والقوسٍ عالٍ بينما قوس القدم الأخرى هابط،  تكون الركبتين متواجهتين. نُسمّي ذلك بالساق K، إذ إنّها تبدو مشابهة للحرف k في اللّغة الإنجليزية

 

 

 

In this example, the leg is straight and the arches are up with the knees facing away slightly. This is what we call the ‘normal position’.

في هذا المثال، الساق مستقيمة والأقواس مرتفعة مع ركبتين متباعدتين قليلًا. هذا ما يُعرف بالوضعية الطبيعيّة

 

 

 

In the next case, this person has weak muscles in their legs from lack of exercise as well as flat feet and that is why the knees have dropped down to the side slightly.

في الحالة التالية، لدى هذا الشخص عضلات أرجل ضعيفة ناتجة عن قلّة التمارين وتسطّح القدمين. لهذا السبب هبطت الركبتين قليلًا نحو الطرف 

 

 

 

For the last case, this is an athlete that has flat feet. They are still able to perform and their thigh muscles are strong and that is why they are able to maintain the position of their knees.

أمّا بالنّسبة للحالة الأخيرة، يتمتّع هذا الرياضيّ بقدمين مسطحتين. لا يزال بإمكانه ممارسة الرياضة بسبب قوّة عضلات الفخذ التّي تحافظ على هذه الوضعية للركبتين

Have you ever wondered how your foot becomes arched like this example when it used to be a flat foot?

هل تساءلت يومًا لماذا أصبحت قدمك مقوّسة بهذا الشكل في حين كانت مسطّحة من قبل؟

 

So much of the time everyone talks about the muscles in the feet, particularly this muscle here. 

دائمًا ما يتحدّث الجميع عن العضلات في الأقدام خصوصًا العضلة هنا

 

When this specific muscle tightens it lifts the foot arch up, however, it does not keep the arch up for very long as it fatigues and drops down.

عندما تشتدّ هذه العضلة بالتحديد، ترفع قوس القدم. الّا أنّها لا تُبقي القوس مرتفعًا لمدّة طويلة لأنه يتعب ويهبط  

 

The underlying ligaments are the secret to lifting the foot arch and aligning the ankle back to straight.

تُعتبر الأربطة السفليّة السرّ لرفع قوس القدم واستقامة الكاحل من جديد

 

In this example, these are the ligaments that keep the foot arch up. 

في هذا المثال، هذه هي الأربطة التي تُبقي قوس القدم مرتفعًا 

 

When lifting the arch of the foot up, we do not simply concentrate on the muscle as the ligaments keep the arch up in the long term. 

عندما نقوم برفع قوس القدم، لا نركّز فقط على العضلة بل على الأربطة أيضًا حيث أنّ لها الدور الأساسي في إبقاء قوس القدم مرتفعًا على المدى البعيد
